Release and Waiver
By completing this form, I hereby acknowledge that I have voluntarily applied to participate and use the CANNABIS AWARDS MUSIC FESTIVAL skate park area. I understand that extreme sports such as skateboarding, rollerblading and scootering involve risks of injury to me and other individuals. Risks include, but are not limited to, disability (physical and/or mental), temporary or permanent paralysis, damage to equipment and/or personal property, and death.
I fully understand the CANNABIS AWARDS MUSIC FESTIVAL, All Access Vegas LLC, and Backstage Bar and Billiards / Fremont Country Club assumes no liability for damage, loss or injury of any kind sustained by myself or property, while using the CANNABIS AWARDS MUSIC FESTIVAL skate park area.
I as an individual assume all liability associated with using the skate park area. By acknowledging and signing this release of liability, and by using the CANNABIS AWARDS MUSIC FESTIVAL skate park area, I hereby fully release and discharge CANNABIS AWARDS MUSIC FESTIVAL, including their staff and volunteers, host, venues, etc. from any and all liabilities, claims, demands, damage, and rights of action or causes of action present or future, resulting from or arising out of my use or intended use of the said area and equipment.
